Our school is taking part in the Michigan profile for healthy use Survey supported by the Michigan Department of Education and Community Health. The research survey will ask about the health behaviors of 7th, 9th and 11th grade students and will include questions regarding nutrition, physical activity, and substance abuse. It will also ask about sexual behaviors. Students will have the opportunity to take the survey on December 14. If you object to your child’s participation in the survey please complete the opt out letter that is available on the link below. You will only need to return the form if you do not give your child permission to take the survey.
